Start with the CBC. Identify whether red cells, white cells, platelets or multiple cell lines are abnormal.
Classify the red cell pattern. Use MCV, hemoglobin and the reticulocyte response to frame an anemia.
Look for hemolysis. Integrate bilirubin, LDH, haptoglobin, reticulocytes and smear findings.
Evaluate bleeding or clotting. Use platelet count, PT/INR, aPTT and the clinical bleeding pattern.
Examine the peripheral smear. Look for cell morphology, abnormal precursors, fragmentation and inclusions.
Use marrow and molecular clues. Later clues may include bone marrow, cytogenetics, immunophenotype and genetic findings.
